Do I have to apply for Medicaid or Medicare?

Yes, if requested by our social worker, you must apply for Medicaid/Medicare within 3 months.  This is to ensure that you receive insurance coverage if you are eligible for it; in order to cover your future medical costs in the community.

What should I bring to my appointment?

1. Your Medications (in their original containers).
 
2. Proof of Income (in order to qualify for no-cost blood tests or X-rays from our partner Sentara RMH).  This can be pay stubs for the past 30-days or your most recent tax form PLUS any disability support, child support, or alimony payment information.

Will the clinic be open during bad weather?

Please check the local TV station (WHSV) for listings of closures that may occur due to bad weather.
