Yes, if requested by our social worker, you must apply for Medicaid/Medicare within 3 months. This is to ensure that you receive insurance coverage if you are eligible for it; in order to cover your future medical costs in the community.

1. Your Medications (in their original containers).

2. Proof of Income (in order to qualify for no-cost blood tests or X-rays from our partner Sentara RMH). This can be pay stubs for the past 30-days or your most recent tax form PLUS any disability support, child support, or alimony payment information.
